Bexley Podiatry: Foot & Ankle Clinic ("we", "us", "our") is committed to protecting the privacy of your personal and health information. The information we collect

To provide podiatry care and run the practice, we collect personal information such as your name, date of birth, contact details, Medicare, DVA, NDIS and private health fund details, and - where relevant - your GP or referring practitioner.

As a healthcare provider we also collect health information: your medical history, the reason for your visit, examination and assessment findings, imaging and test results, treatment provided and clinical notes.

How we collect it

We collect information directly from you - in person, by phone, or through forms on this website (for example the appointment-request and referral forms). With your consent, we may also collect information from your GP, specialist, other treating practitioners, or your NDIS plan manager.

How we use your information

  • To assess, diagnose and treat your foot and ankle condition and provide ongoing care.
  • To communicate with you about appointments, results and your treatment.
  • To liaise, with your consent, with your GP, specialists and other members of your care team.
  • To process Medicare, DVA, NDIS and private health fund claims.
  • To meet our legal, professional and record-keeping obligations.

Disclosure of your information

We do not sell your information. We disclose it only where necessary for your care or where required or authorised by law - for example to other treating practitioners you consent to us involving, to Medicare or your health fund for claiming, or to comply with a legal obligation.

Storage and security

Your records are held securely, whether in electronic or paper form, and access is limited to staff who need it to provide your care or run the practice. We take reasonable steps to protect your information from misuse, loss, and unauthorised access, modification or disclosure.

Access and correction

You have the right to request access to the personal and health information we hold about you, and to ask us to correct it if it is inaccurate, out of date or incomplete. To make a request, contact the practice using the details below.
